Malgré les progrès de la pharmacologie, le traitement de la maladie mentale demeure un défi et nous confrontons encore trop souvent des résultats insatisfaisants avec les agents existants. Le rétablissement se définit par la capacité de fonctionner en société, de participer activement à des activités significatives et de maintenir des liens interpersonnels enrichissants, tout en demeurant relativement libre de symptômes. Les patients veulent se réaliser et s'épanouir et l'atteinte de ces objectifs requiert plus qu'un simple équilibre entre efficacité thérapeutique maximale et effets secondaires minimaux. Une approche thérapeutique individualisée doit tenir compte des symptômes, des comorbidités, de la réponse aux traitements antérieurs et des effets indésirables, mais aussi des choix et des attentes des patients. Il faut trouver l'équilibre entre les objectifs et effets du médicament à court et à long terme. Le choix du traitement doit permettre, voire promouvoir la capacité des individus de réaliser leurs aspirations.
